Sickness Certificates
You do not need a doctor’s sickness certificate for any illness lasting seven days or less. Your employer may need you to complete a self-certification form (SC2). This is available from your employer or on the HMRC website.
Sick note for more than 7 days
To request a sick note when you have been sick for more than seven days:
- Fill out a sick note request form online – Request a sick note using PATCHs
- Phone or visit the surgery during opening hours
- Sick notes can only be issued when they are due – they cannot be future dated.
- Sick notes can be back dated.
